No Patient-Provider Relationship

Use of this website, including communication via contact forms, emails, or comments, does not establish a patient-provider relationship between you and Empower PhysicalTherapy Healthcare services are only provided after a formal evaluation, signed consent, and agreement to treatment and patient intake form have been signed by prospective Patient. In accordance with New York State physical therapy laws, direct access physical therapy services without a physician referral are available for up to 30 days or 10 visits, after which a referral is required.

 

Telehealth & Third-Party Technology Disclaimer

• If telehealth services are requested by a patient and provided by the Provider, they are subject to New York State telehealth regulations (Public Health Law § 2999-cc). By engaging in telehealth services, you acknowledge:

• Third-Party Digital Platforms – Telehealth sessions may be conducted via HIPAA-compliant third-party platforms, but Empower Physical Therapy does not own, control, or guarantee their security.

• Limitations of Telehealth Care – Telehealth is not a substitute for in-person care where physical assessment or manual therapy is required.

• Privacy Risks – Although efforts are made to ensure privacy, telehealth communication may be susceptible to breaches beyond our control.
